2023 North Central Consultation Information- *All Digital Forms Continue This Year* Due Nov 1st

The 2023 Consultation Forms are now available!
Please note the North Central District churches are not turning in the paper forms this year.
Last year the North Central District made the consultation form process entirely digital. We are continuing this trend as North Central is the first district to try this process in 2022, and wish to keep up with this, so we invite you to offer your feedback throughout. There will be a feedback survey in November on your thoughts for 2024.
Once you’ve completed the form, you will receive a confirmation email to the address you indicated on the form. To collect the other signatures required for your form, forward the confirmation email to the other individuals who need to sign. Ask them to click the link in the email and select the "Edit Form" button in the upper left once the page loads. Once they add their signature, they will need to click submit on the form.

- 2023 Appointed Associate Pastor Consultation Form (Completed by Associate Pastors) Must be filled out by the Associate pastor and signed by the lead pastor and PPRC.

Every pastor serving under an appointment is required to complete a Consultation Form. The PPR at each church is required to complete one as well. Please fill these out as truthfully as possible and have those conversations with your PPRC before submitting. Make sure to have both signatures.
The deadline to submit the forms to the district office is November 1st. It is important to meet this deadline to give the DS a chance to review the forms and set up the necessary meetings in preparation for Cabinet meetings in January. This deadline is also important because of our new district size.
